This page explains the information collected through the DPF Utah website and the purpose of the service request form.

Information you provide
The service form collects your name and a phone number or email address so the shop can reply. It also collects your customer type, the service requested, whether the filter is installed, and the message you send.
You can add a company name, vehicle details and location to help explain the job. Send information relevant to the service request. The form does not ask for payment-card information, financial account details or identity documents.
What the inquiry is used for
Your inquiry is stored privately for handling the service request, understanding the vehicle or component involved, and coordinating the next steps. It is not published on the website.
Agreeing to contact on the form means contact about that request. It is not a signup for marketing messages. Submitting the form does not make a payment or automatically book an appointment.
Technical information
The website keeps a request identifier, submission time and originating page path with the inquiry. These help identify a request and prevent a repeated submission from creating duplicate records.
To limit automated abuse, the intake system also stores a protected fingerprint derived from the connection’s IP address. The inquiry record does not store the raw IP address. The services that host and protect the website process connection information needed to deliver it.
The website does not currently add advertising trackers or behavioral analytics. Following a link to an outside website, such as a map or a technical source, takes you to that provider’s service and privacy practices.
Ask about your information
To ask about an inquiry, correct a detail or request deletion of information you submitted, contact DPF Utah or call (801) 268-0919. Include the request reference if you have it and a way for the shop to reach you.
The shop may need to confirm which inquiry belongs to you before acting on the request. You do not need to send payment information or identity documents through the form.
