What should I send with my request?

Is cleaning guaranteed to fix the warning light?
The quote should address the diagnosed problem. A vehicle warning can involve more than the filter, so ask what diagnosis and final vehicle checks are included. Component cleaning alone does not establish that the original fault has been resolved.
